I tried to uninstall taxonomy_module, then when I went back to 'site building -> blocks' the block created by taxonomy_menu was still there.
I installed again figuring it was me not "defaulting" the module before disabling. I changed the settings back to 'none' saved then disabled. I also used the uninstall feature. The block still shows.
I also located the block id (74) then went into the database and removed the row associated with it. The block shows still shows on the page.
How do I get rid of it?

hutch commentedSounds like a cache somewhere, D5 does not cache blocks AFAIK so it could be your browser or you have caching switched on. Go into blocks and save, Go into menu, edit a menu item and save
