No combo box - no menu items showing

I am using the version posted here (uc_catalogmenu-5.x-1.0.tar.gz), and have no combo boxes to select the categories. Mine looks the same as the comment by Spacefairy on the Ubercart site: http://www.ubercart.org/contrib/3202#comment-21030
When I try to use the ID in the text box nothing shows up in the menu. I am using Pathauto, but I can't think of anything else that might be conflicting. Do I have the right version?
This is exactly what I was looking for, so thanks for your contribution and I hope I can make it work.

This is the right version to use. Is it possible to check out your work if it is in internet. Otherwise i need to know your drupal and ubercart version that you are facing problem.

I'm having the same trouble with this module. Could anyone, please, solve this issue? The module seems to be a useful one.
