Is it something that make sense to let user choose if they want to be notified by email, by sms, or on their profile page for example ?

If you check out #361550: Planning the next version of activity (6.x-2.x), you'll see that I'm actually thinking about using Messaging/Notifications as the back end for the next version (though D6) of Activity module.

Definitely, no new features for 5.x

About 6.x, we'll see how it can be integrated with activity 2.x, possibly we'll be adding feeds and/or trigger/actions support so there will be a workaround for that.
