I wonder if someone would be good enough to clarify where the 'Setup Database' button is located.

Point 3. in the installation section of the Install.txt states the following:

After enabling this module in the previous step, navigate to
administer > access control > simple access
and click the 'Setup Database' button. The module is now installed"

However the menu path is incorrect.

What is the exact location of the 'Setup Database' button? Could someone possibly provide the URL for this?

This is old documentation. I have now corrected this in dev.

Basically you need to "Rebuild permissions" which is in Administer > Content Management > Post settings
