I followed all instructions and also comments in the bug report area to configure the module. However, althought the attachment on top is enabled, the area for attaching files to a page does not appear. How is possible to solve this problem?

A little comment: more information should be included in the INSTALL or README files.

The problem was in the access rights. I solved it. However documentation should be improved...
