Add a country select for block configuration

Like i18nblocks does, we need a configuration option to limit a block to a given country or countries. (We shd accept multiple values--no reason not to, not sure why i18nblocks currently accepts only a single language.) We'll need to implement db_rewrite_sql(). See the implementation in i18nblocks, as well as the form_altering of block configuration.
