Default image for anonymous comments missing

Description
Gravatar installed and default image set. If a logged in user with no gravatar comments then they get this image. If an anonymous user comments then they get a broken image link. The link in the tag is to the root of the domain the site is on.
Not sure if this is a misconfig or a bug - any hints appreciated.
