Issue Summary
E-mail gateways are currently hard coded into the E-mail Gateway module. We should move this to the database and allow users to add, remove, and edit these settings.

Was looking to beef up one of these, but I'm only interested in this time in having US/CA based carriers on my site so didn't want the patches without a db way to enable/disable certain countries or carriers directly.

I haven't used these email gateways, but I think they are editable in 2.x
