Also, regarding 5., in my /themes/garland directory, there is an advforum subdirectory, should this be removed in the new version? I should just copy over the naked directory from /sites/all/modules/advanced_forum/styles into /themes/garland?

Yes, you can delete the old advforum directory. That's been replaced by the new styles. Just copy over naked, if that's the one you want to use.
