Looking for one module created - have form interface already + logic written out in plain english

We are looking to have a module created to send a user to a product page (ubercart) based on the values in their form. There shouldn't be any need for database useage for this - just a form module that checks the conditions based on the entered values and then sends the user to one of 20+ different product pages

Will also need to be able to send the values entered in the forms onto corresponding text fields in the product node (which we can do if we know how to access the variable in the product node template)

Linked is a file including the conditional statements written out + the forms already created (uses javascript to switch between forms), and the reference chart from which the logic was created.
