flag_gb.gif is missing
"User from gb Great Britain (UK)"

Ip2nationCountries table is using 2-letter iso codes now. You can find UK flag as flag_uk.gif. You can rename it to flag_gb.gif.
In the next release, I will use famfamfam flag:
http://www.famfamfam.com/lab/icons/flags/
I think they provide more accurate country flag using 2-letter iso codes.
