Flag paths are like this:
example.com/en/sites/all/modules/ip2nation/flags/flag_gb.gif
so flags do not show up because of wrong path.

I was using German as default language and english as second. I did not use image-cache etc. But i alreay uninstalled that module so i can not give any further informations. sorry.
