Where the image used to be it now says "Add a note" in plain text. That's it.

do you have a public link that can be viewed?

I haven't encountered any problems like this so far

I see that some imags have the "Add a Note" text (no link), then a duplicate piece of the image then the image. Some images do that in Firefox.

Some images don't show on Firefox but show the above problem in IE 8.

Drupal 6.6.

I'm not the original module developer so I can't speak to the methodology for making image annotate work but in my testing it does work. Here is my sample page:

There is now a beta version of the module; please give it a try, I will post a stable version in the next few days.
