I don't think this is a FlashVideo issue but either an Upload module issue, or a browser issue. The FlashVideo module is not responsible for attaching the files to the node, but just takes over once the files have been attached.

wojtha commentedIt's probably upload.module issue ... I've found this error searching for error caused by upload module directly (Drupal 6.10).

I installed the jquery update module and its fixed.
Install the jquery update from:
http://drupal.org/project/jquery_update
