Under 'general settings', the following setting:

Update action:
Do nothing. Leave the old alias intact.
Create a new alias. Leave the existing alias functioning.
Create a new alias. Delete the old alias.
What should pathauto do when updating an existing content item which already has an alias?

If the first choice is selected, no aliases will be created even if there is NOT an alias to begin with. This also happens during bulk updates.

This happens in the 6.x.-1.1 and both of the listed 6.x devs.
