How to make a "Recommend this website to a friend" page?

Hi,

I want to set up on my Drupal 6 website a simple "Recommend this website to a friend" page, as shown in these (non-drupal) examples:

Do you guys have any idea how to go about doing it?

P.S.: I want to append a small link to this page on emails sent to my users.

There is a module for this : http://drupal.org/project/tellafriend.
I 've never use it though...
