I've deselected the sub-administrative area and the dependent locality, but they still show up in the node entry form. I've deselected these in the Geocoded Address Marker field widget, as well as in the Google Map Tools Configuration. No luck.

Also, wondering, how to change the name in the entry form to City, State, Address, and Zip

This feature is not yet supported.

The first(widget) block affects the generated "taxonomy" terms (DISPLAY ONLY!!!!!!)

The second(settings) block affects the generated menu tree.
