Please list:
version of drupal in use? (in future please tag your thread with the version)
version of audio.module in use?
type of file being used? .mp3? .wav?
bit rate of the file?
it seems flash only likes certain bit rates from my own research when playing with audio files and flash players.
size of the file being used? Is it a very large file? does the file actually exist in the folder you uploaded it to? is the file the exact same size as that which you uploaded?
By doing the above those with the ability can set up a quick test on their own machine see if we get the same issue happening.
something else to consider is placing a link to your site so that the behavior can be witnessed and better understood or investigated. Though I realize doing so is not always possible.
There is no audio.module ready for 6.x yet. It's correct version is listed as 6.x-1.0-unstable3. As seen on the project page = http://drupal.org/project/audio
If you don't have the skillset to fix the bugs in it, you should not use it as the developers are still working on porting it.
-unstable = do not use unless you are a developer willing to fix bugs
-beta = ready for beta testing
-rc = release candidate and is very close to being ready to use on production sites, and in some cases you can deploy
