I've tagged a DRUPAL-5 branch and started work on a Drupal 6 port. I'll be posting a link to the initial commit in HEAD once I get this issue ID.

Committed in #157555. This code will not work - I haven't touched anything with actually creating the Book nodes, as the entire system was rewritten for D6. The {sequences} table is gone, as well as the specific parent / child relationships, as it just uses the menu system instead.

The module is now in a state where it seems to work in Drupal 6 with light testing. I'm able to create a skeleton and instantiate it with the hierarchy being preserved properly. This issue will now be marked as fixed, with any new tasks being opened as separate issues.
