HTML In Node Reference Select List

I have an issue where there is HTML code showing in my Node Reference select list. This doesn't change from single or mult-select. It formats it like this: <ul><li>Node Title </li></ul>. I've included a gif of it as well.

This only happens on node/add pages on all node references. Once you go to edit a page the list is fine again.
