Basically, its not recognising [cat] or [cat-path] as valid tokens, and just prints them in the title verbatim.
Of course, now that I look closely, that's exactly what it's supposed to do.
[Cat] is only valid within a taxonomy term scope, not on a node page, where I was doing it.

Bear in mind [cat] will only be relevant in the appropriate scopes (such as node or term pages).
