Modify the handbook contributor lists

Description

Pursuant to the changing definition of the docs team and recent discussions on the mail list, we want to combine the "contributor" and "editor" pages into one page. We can also remove the count of edits as well and just sort by most recently edited. I've got a patch to drupalorg coming.

OK, this patch:

- removes the "Revision maintainers" case (node/263594 can be removed after the patch goes in)
- removes the old book_contributors function
- modifies the revision_maintainer function to be the new book_contributors function
- removes the count stuff and just sorts it by last revision
- changes the cache ID to the right node ID
- I also removed the lead text on the Maintainers page so that we can edit that text as we like on the node without having to deal with patches to the module.

bumping again. please. we are publicly defining doc team differently but the handbook page is still old skool. or would you prefer that we try to do this with a view instead?
