The 404 page has no title. The attached patch adds a title of t('Page not found'). It also removes the line $output = '<p>'. t('The page you requested was not found.') .'</p>'; which is made redundant by the page title and the drupal messages. I also simplified the usage of the $output variable in that function as a result of those changes

There are 2 other translations to be included which is why I have not yet put it into release yet.
