Ability to enter a reason for a flag

Description
In the Drupal 5 version of Flag Content users could enter a reason for flagging a certain node. I recently upgraded to D6 and now this feature is no longer present.
I'd really like to see that on D6 :-)

I haven't upgraded from 5.x to 6.x yet, but we are planning to upgrade soon. This is a great feature on the 5.x version.

Here is a quick patch to add missing "reason", "name", and "email" from 5.x
