Extend form_builder_example.module

I'd like to extend the example module to include the following three examples.

1) A contact form.
2) A registration form (possibly mimic a DrupalCon registration).
3) A simple survey form.

I believe this will enhance the initial impact this module will have on non-code users as to what can be done with the module by providing more real-world examples of how to use it.

I categorized this as Documentation because that's basically what the example module is.
