Integration with token

Description
I was wondering if or how it would be possible to integrate String Overrides with Token so that the following replacement could be possible:
"@username's blog" -> "[user-profile-real-name]'s blog"
If this isn't possible, could you please point me in the right direction? I can code my own module if there's no solution.
