I am... stupid! Sorry for that guys. This is fixed along with a couple updates in the latest versions of the module (6.x-1.1 & 5.x-2.1) that should be available shortly (as soon as the packaging scripts run again).
Yes the table is needed: you may have several picture or no picture at all suitable to build your thumbnail. The table stores the original.
I just tested again on a fresh Drupal 6 install: it works fine on my side. Please follow the instructions from the README.txt file step by step and share your feedback if you still have issues. Make sure as well to properly uninstall your modules if you do testing.

I installed 6.11 but I see no change. I tried saving and updating a node but nothing gets inserted in the teaser thumbnail table. Any tips perhaps? :)
Is this table needed at all? I see it has columns for nid and filepath, but I guess if an imagepreset is used this seems unnecessary. :)
