Taxonomy Access Install No Longer Working

I tried installing Taxonomy Access Module. Uploaded it to the modules directory. First problem is that the module is not listed by name on the list of modules. A blank space exists with the checkbox. Upon installing, one gets a database error that the tables cannot be created. If one manually creates the tables, you cannot administer the module from the modules administration page.
