I have installed the CAPTCHA module on my 4.6.0 drupal. I installed the comment.module patch. The only patch that seemed to work for me was comment.module_9.patch. Everything worked. Now it doesn't show up anymore. I have tried all the other patches and still doesn't even appear. My board is being spammed heavily.

Patches were for older revisions, maybe you can try patching by hand? Closing for the moment, please reopen if you still have this problem. I'd recommend upgrading to latest drupal (4.6.x or 4.7.bx) first.
