Issue Summary
When the node type setting for creating menu items is set to "optional_yes", and you choose NOT to create a menu item for a node, and then edit the node the "Create Menu" checkbox is checked (it should not be).
The following patch fixes this so that when you edit a node, the optional_yes setting does not override your initial deselection of the Create Menu setting.
