I need cost-sensitive quotations for integrating the following functionality into an existing Drupal 6 website:
Basic description:
The idea is to have user fill in a online form and tick some checkboxes. After submitting they will receive an email with a customized attachment (pdf, if possible) and the site's admin will receive a notification.
The idea is to provide an automatic quotation of professional services to the user and pre-fill a form that they would have to sign.
The form:
a) Mandatory fields: e-mail address
b) Optional fields: name, address (with a small explanatory leyend that the optional details will customize the output of the submission)
c) Checkboxes with the name of different countries
d) Checkboxes with a description of the services to be quoted

The form's output
I will provide a list for all the services in each country and their respective cost. This will serve as the basis for the form's output.
In addition, each country will have a separate document that will need to be filed out with the personal information entered by the user (name, address).
In the end, the user will receive a list of costs and an accompanying customized document for each country, ready for printing (thus the need of using pdf's, although this is flexible).
