There was a bug that has been fixed in 6.x-1.x with regards to the duplicate entry with src = 'taxonomy/term/'. Please download the latest version and report back if this is still an issue. Marking as a duplicate, but I can't find the original isssue.

Duplicate of #278368: taxonomy_term_path being passed TID instead of Term Object. Upgrade to 6.x-1.x-dev.
