BookMadeSimple & OG User Roles Bugfix

It's not really a bms bug, the problem was with the ogur code, but it will appear to people as if it's a book made simple bug so I'd thought I'd cross reference my ogur issue here as well.

The problem is that non-book content types don't get inserted into the book outline when og_user_roles is enabled.
