Add option to control default collapsed state of permissions fieldsets

Description

I have added an option to admin/settings/util/apfm to "Collapse all permissions fieldsets by default".

I have also change the TODO text, it now reads:

TODO

  • Add per-module permission fieldset collapsing/expanding
  • Add per-role permission counter (administrator[3/10])

I have left the default state as it was before (expanded).
