Add option to control default collapsed state of permissions fieldsets

Description

I have added an option to admin/settings/util/apfm to "Collapse all permissions fieldsets by default".

I have also changed the TODO text, it now reads:

TODO

  • Add per-module permission fieldset collapsing/expanding
  • Add per-role permission counter (administrator[3/10])

I have left the default state for permissions fieldsets as it was before (expanded).

Not only do you get the setting to collapse all, but also the ability to collapse by module.

Committed to 6.x-2.x-dev.

Still need the count by role, if you think that is really a good idea (some sites have tons of roles).
