How to make a "report this page" link on each page?

I'm confident there are about 1800 ways to do this, but I haven't figured out any simple way to do it.

On my site, I'd like a simple text link in the footer block that says something like "Please Report Errors On This Page" or something like that. If the user clicks it, they get a little text box that allows them to complain about something on the page and optionally put in their email address.

If they are logged in, I'd like the email to automatically include their username. In any event, I'd like the email to automatically include the URL of the page they are currently on.

Can I use the core Contact Form module to do this? If not, what would be a simple stable way to do it?
