I ran into this after saving the a comment without preview. This happens only for forum topics being added to a book. Stand alone forum topics do not have this problem.

Preview and save works fine.

Don't know if this is related to comment or book modules.

Use comment_notify 6.x-1.x. The bug has been fixed there.
