Enable path module
Create a node, selecting parent menu item but leaving menu title empty
Save the node and it's not entered into the menu
Proposed solution:
Add an item to the parent select options "No menu entry" (or something similar)
On submit set the menu title to node title of menu title is empty
Only add a menu item if the parent is not set to "No menu entry"
On edit, if no menu title is set default it to the node title.
