Cannot upload larger 166MB file

I can upload up to 80MB file but cannot upload 166MB. When I try to upload this 166MB file, I get a "access denied" to the "swfupload" path in the log page. The weird part is one time the "access denied" user is "Anonymous" while I was logged in. I can see the "Session opened for..." log right before the "access denied" log.

I try to test this on my local development machine. I don't see this "access denied" log but the SWFupload just the stuck at the very end and never finish.

Any tips on how to debug this problem?

I did a view source on the page and I can see the SWFupload javascript settings, the post_params is there with the sid etc.
