This apparently is an Opera only issue, other browsers it works fine.

Is there some preference in Opera that is preventing External Links from opening new windows?

Ok think I figured it out, it was the Opera preferences, more precisely, Opera main block popups does not sync with the individual edit site preference block popup.

In order for it to work properly, the main preference block popup has to be set.
