Create Formatter Fieldset in Media Browser Form

This will be a form just below the file selection form, initially hidden, where the user can enter things like 'List file' and Meta data. It will need to have vertical tabs based on mime type (Image/Video/PDF/etc), and will be automatically opened and selected on original submit (but the user can override if it selects incorrectly).

Mockups would be great, if someone wants to fill that out more before we design the forms.
