I'm about ready to do an upgrade. When it says remove old files, I have some additional files in my current directory that I'm not sure if I should delete. They are cgi-bin folder, a tmp folder, and a thumbs.db file. Any harm in leaving these?

Not at all
cgi-bin and tmp are folders that the server uses, don't delete them. Thumbs.db I have no idea, i would leave it though or make a backup and delete it, see what happens.
Upgrade in one step from 6.4 to 6.10
I need to upgrade several sites from 6.4 to 6.10. In the past, I seem to recall having a bad experience (ie. trashed SQL tables) if I did this big a leap in the revisions. Is there a guideline on when not to do this (and instead, upgrade sub-release by sub-release (e.g. 6.4 -> 6.5 -> 6.6 -> ...)) ?

I kept the extra files and did the update, it went as smooth as silk, no problems.
