I did accessibility check on WAVE web site (http://wave.webaim.org) and the search from this module came as an error.
It says
ERROR: Form error missing
A
or does not have corresponding label. Lables are not required for hidden, image, submit, , reset or button form elements.

I don't see how this is a bug report. You have not described a problem or suggested a patch. I am not interested in output from your software which seems like incomplete sentences with values missing.
