I found a need to create an additional field for one of the roles. I went to admin/profile/user/manage and created the field, a single line text field. When I checked the account request page, the field was not present. I tried clearing cache, but it didn't make a difference.
I tried creating another field for another profile, but it did not show in the request-account page, either.
I have attached screen shots showing the profile/manage page with the field profile_youth_names, the profile/role page, and the request-parent-account page.
