I just deleted pathauto and downloaded the latest 6.x-1.dev today April 28 and I still get the following when pathauto is enabled but not when it is turned off.

INSERT INTO url_alias (src, dst, language) VALUES ('node/127', 'content/storey', '')
INSERT INTO url_alias (src, dst, language) VALUES ('node/127/feed', 'content/storey/feed', '')

I am running Drupal 6.8,
Pathauto 6.x-1.x-dev (2009-Apr-20)
Panels 6.x-2.0-alpha3
Views 6.x-2.5

I did have Tracker enabled but I turned it off still no difference.

Any ideas? I saw that I needed the latest dev version of pathauto but that didn't help. Any time I edit a node I get similar errors.

Not a Pathauto issue: #269877: path_set_alias() doesn't account for same alias in different languages
