Images not showing on random pages

Just upgraded to the lastest stable version and am getting some weird issues.
I have a tabs block that contains: Popular for Day and Most Popular

These are views block and loaded through a php block.

On the first tab I have an article that should show an image and the article name.
Ont the second tab I have the same article that DOES show the image and article name.

So I know I setup the views correctly. It's weird, my taxonomy images are colored dots. So I have random dots now.

Another weird thing is I'll be on a page and look at a related articles block and there will be an article named: XYZ, I go to another article and the related articles block will have the same article XYZ, but the taxonomy image will show up.

Whats going on?

I tried going through and re-adding all the images, I've cleared my caches over and over.
I've go to every article, checked my folder permissions.
