empty page if CCK modules are disabled

Trivial patch: we ditch the old hook_page() for a pure menu-based hook. Without this patch, the flexiconvert menu yields an empty page that doesn't even show the warnings, it's very confusing. With the patch, it shows the page with the usual explanations and warnings if CCK is not configured but not the form.
