Hi,

when there are no commentators subscribed to a node, the array $alert in _comment_subscribe_mailalert() won't be filled and therefore there will be no !name for the subscribed author of the node. I guess it should be possible to get the name of the author in those two queries in line 380/383.

It has been decided to merge Comment Subscribe module with the Comment Notify module as both currently have the same set of functionalities. We have stopped our support here and will be providing our support at the Comment Notify issue queue.
