FTP access is fine
Shell access is not necessary at all to add, remove, or modify themes. It pretty much isn't needed at all, except for the initial database setup, and than most people will prefer to do it through a GUI such as phpmyadmin or the like.
New wrinkle: shared install
Is there any way a central Drupal install can be used with per-site themes?
