I'm trying to use activity on a D5 site, but every activity generates about 5-10 duplicates on the activity page, rendering the module effectively useless.

How can this be fixed and what's causing it? I've tried using the time filter but see no results from it.

first question: are you using the latest -dev snapshot of Activity?

I think I was, yes. I switched back to the latest full release and am not having the problem anymore. If the dev release is going to be the next full release for D5, make sure to fix this problem! :)
